Intake Process:
Verify there is an open assistance program for your diagnosis, collect your documents, and apply online or call. Caregivers, pharmacists, and providers may apply on a patient's behalf. Required information includes: diagnosis and medication name; patient demographics: Name, address, Social Security Number and phone number; income: adjusted gross income applicable for the patient and all members of the patient's household; insurance: health insurance and pharmacy card(s); and physician demographics: prescribing physician name, phone number and facility address.

Eligibility:
The patient must be getting treatment for the disease named in the assistance program to which he or she is applying; must have health insurance that covers his or her qualifying medication or product; patient's medication or product must be listed on PAN's list of covered medications; patient's income must fall at or below the Federal Poverty Level specified by the assistance program; and the patient must reside and receive treatment in the United States or US territories. (US citizenship is not a requirement).
