Intake Process:

Must complete application and provide all required documentation and be screened prior to receiving an appointment. Applications are available on our website or at our clinic.

Program Fees:

Medical Patient Visit - $25
Medication Administrative Fee - $5 per medication ($35 cap per visit)

Eligibility:

Must reside in Northeast Tennessee or Southwest Virginia without access to affordable medical care. You or someone in the household must have been employed in the last 12 months or you are a student. Must have no medical, TennCare/Medicaid or Medicare Insurance.

Total household income may not exceed 250% of the federal poverty guidelines:
Monthly:
Family of 1 person: Household income $2,831
Family of 2 people: Household income $3,815
Family of 3 people: Household income $4,798
Family of 4 people: Household income $5,781
Family of 5 people: Household income $6,765
Family of 6 people: Household income $7.748
