8:00 AM- 5:00 PM; closed for lunch from Noon-1 PM

Intake Process:

Must complete application and provide all required documentation and be screened prior to receiving an appointment. Applications are available on our website or at our clinic.

Program Fees:

Dental Patient Visit - $50
Dental Hygiene Visit - $25
Oral Surgery Visit - $100
Dentures - approximately $400-$600

Eligibility:

Must reside in Northeast Tennessee or Southwest Virginia.
You or someone in the household must be employed OR You are retired OR You are a student OR You are Legally disabled.
Must not have dental insurance.

Total household income may not exceed 250% of the federal poverty guidelines:
Monthly:
Family of 1 person: Household income $2,831
Family of 2 people: Household income $3,815
Family of 3 people: Household income $4,798
Family of 4 people: Household income $5,781
Family of 5 people: Household income $6,765
Family of 6 people: Household income $7,748
