Provides services targeted to advance independent living, development or maintenance of abilities, and greater accessibility for people of all ages with a variety of disabilities, with primary focus on physical and developmental disabilities.
Services include durable medical equipment, wheelchair ramps, family support services for persons with disabilities in Rutherford County, home modifications, recreation, and education & leadership development programs. Works with the TN Council for Developmental Disabilities to provide grants for travel to workshops and conferences for persons with disabilities and their families/caregivers and offers Christmas boxes with gift cards and other items for current needy UCP clients.

Description
Seasonal activities for people with disabilities and their families that takes place on Thursday evenings and during specially designated weekends. Activities include wheelchair basketball, bowling, picnics and special outings. This is a program run entirely by volunteers.

Description
Working through a statewide partner network, donations are allocated to meet specific home and community based housing needs of low income persons with disabilities in Tennessee who do not receive housing supports through state Medicaid Waiver programs. Depending on specifications of the donor, the overall fund may be used for a variety of purposes specific to disability related housing needs. However, it is important to understand that some donors require that funds must be spent on housing modifications, so funding may not be available for all aspects of the program at all times. The fund is used for small construction activities specific to disability needs such as widening doorways for wheelchair access, modifying bathrooms, and installing grab bars. Typical funding is $5,000 or less.

All Applications: Please enclose the following attachments: 1. Documentation of Disability â Letter or other document from a physician, therapist, school personnel or other professional that verifies that the application has the disability described in the application 2. Functional Limitations Form â Complete the enclosed form that describes how the above disability affects the applicant 3. Complete and sign the Household Member Income form. All household members, regardless of age, must be listed. If the member has no income, write zero on the appropriate income line. 4. Documentation of Household income such as a Social Security benefits letter, bank statement, pay stub, or current tax return for each member listed. Applicants requesting Home Modifications: 1. Permission for Home Modification form. This form must be provided whether you or someone else owns the home.
Eligibility
Eligibility for the fund was established according to the same basic criteria as the stateâs Family Support Program. UCP must document that applicants have a disability that results in a substantial limitation in three or more major life activities and the disability is long-term or likely to continue indefinitely. Individuals must also meet the family income guidelines established by THDA. These guidelines are based on total household income and the number of people living in the home.

Description
UCP provides durable medical and adaptive equipment to people throughout Tennessee, regardless of their diagnosis. UCP seeks new and gently used durable medical equipment to redistribute the donated items to individuals who have little or no insurance and no other resources to obtain the equipment they need to live their lives as independently as possible. If the item needed is not available, UCP provides an application to place individuals on a waiting list. The traditional medical items consist of wheelchairs, scooters, grab bars, walkers, bath benches, toilet chairs, specialized strollers and car seats, bed rails, prone standers, gait trainers, etc.

Description
The Rutherford County Family Support Program was established by the TN State Legislature with agencies serving each county in Tennessee. UCP provides this service for Rutherford County. The program provides financial assistance to individuals with severe disabilities to live successfully in their homes and communities.

Eligibility
To be eligible for Family Support, the individual must live in Rutherford County and have an intellectual or developmental disability that significantly impacts their abilities to perform daily life functions. The person must be living in the community in a home, apartment or other setting that is characterized as an unsupported setting, meaning that no personal support or direct living assistance services are being supplied by state or federal funding.

Builds ramps and coordinates state-wide construction of wheelchair ramps for individuals with mobility disabilities whose homes are without proper accessibility.

Completed application, income documentation for all household members, permission to construct a wheelchair ramp signed by the homeâs owner, Permission for Home Modification form, and signed release.
Eligibility
The program serves Tennesseans with any disability that results in mobility impairment and the need for a wheelchair ramp. Applicants for THDA funded ramps must also meet the THDA household income criteria. Please be aware that due to funding limitations, there are waiting lists for this program.
