Call (901-250-4004) or email (snap.benefits@acc.cdom.org) for appointment. MAKE SURE YOU INCLUDE YOUR FULL NAME AND PHONE NUMBER IN EMAIL OR PHONE MESSAGE. Applicants need: social security numbers for all members of the household; photo ID (drivers license, school ID with photo; passport, resident alien card, I-94 card or voter's registration card); proof of residency (lease agreement, utility bill, phone bill or drivers license with current address); income (mortgage payments, property tax, homeowner's insurance, rent receipt, lease agreement); and costs of utilities (MLGW bill)
