Description:

CIS of Tennessee embeds a caring adult, known as a site coordinator, inside each partner school who works with school staff to identify the needs of the school community and its most vulnerable students. Following a rigorous, data-driven model, site coordinators establish a strategic plan and coordinate with community partners such as businesses, health care providers and other nonprofits to provide daily school-based interventions to students in grades K-12 addressing chronic absenteeism, academic failure, behavioral issues, social service needs and more.

Currently serving the counties of: Bledsoe, Clay, Cocke, Davidson, Fentress, Grundy, Hardeman, Jackson, Lake, Lauderdale, McNairy, Morgan, Scott and Van Buren.

Students identified as needing case management supports may apply or be referred for services if they attend a school in which the agency offers services. Students typically have been identified based on chronic absence from school, mental health/social emotional needs, health and basic needs not being met, creating barriers to learning. Description

In addition to programming and services provided to the full student body, CIS coordinators provide case management services for identified students enrolled in his/her school. Case management services include supporting students and families with basic needs, social emotional development,school attendance supports, connection and referral to other service providers for identified needs, etc. Currently serving the counties of: Hardeman, Lake, Lauderdale, McNairy, Van Buren, Jackson, Grundy, Fentress, Davidson, Clay, Bledsoe, Cocke, Hancock, Morgan, and Scott
