Description
A program providing no-cost care management services for older adults who struggle with day-to-day obstacles including limited resources (social, financial, and support) and/or functional limitations and isolation, making living alone safely and comfortably a challenge. Care management helps older adults feel connected, engaged, and valued.

Proof of Income, proof of home ownership/rental agreement, and authorization for services
Eligibility
-Be 50 years of age or older -Not have a care manager from another agency -Have limited income and assets -Need assistance to access services
