Description:

Provides outreach, education and enrollment assistance to sign up children in need to TN Medicaid (TennCare) and CHIP (Children's Health Insurance Program or CoverKids) in Fayette, Shelby and Tipton Counties. TennCare and CoverKids offers insurance to low- and moderate-income Tennessee families with children who need health care coverage. This program is funded by the Department of Health and Human Services, Center for Medicare & Medicaid Services. Staff also provide the Free Health Insurance Enrollments via the marketplace & state of Tennessee. Can also provide assistance with accessing help for specialty health situations. Spanish speaking staff available.

Mon - Fri 9:00 am - 5:00 pm

Call to schedule appointment; requires state issued photo ID, social security card, proof of resident, proof of income, etc.
