Description:

Vaccines are available at the Shelby County Health Department sites (814 Jefferson Ave, Suite 207, 38105 and 1826 Sycamore View Road, 38134) for children aged 6 months - 5 years old as well. Special low dose vaccines have been available for children ages 5-11. Persons under 18 years of age must be accompanied by a parent or guardian in order to be vaccinated. Parent or guardian will be required to show ID and complete and sign a consent form on-site for their child to be vaccinated. Call or book appointment at website or see website for sites that require no appointment. Call to verify availability of Homebound vaccinations.

Provides city of Memphis COVID-19 vaccine information and registration for sites in Shelby County. Go to the website, read current eligibility guidelines, and if eligible, pick a site and date and dose, and click on the Sign up Genius link by each site. Appointment sign-up links will now be released on Fridays at noon for appointments for the following week. You will receive an email confirmation after registering online. May also call (901-222-SHOT or 222-7468) to make an appointment (after March 12, 2021, this number will roll over the state TN Dept of Health COVID-19 site to make an appointment).
