NOTE: AS OF 7/01/21, IN PERSON SERVICES ARE STILL BY APPOINTMENT ONLY DUE TO COVID-19. Provides Medicare supplement insurance and Part B premiums. The Qualified Medicare Beneficiary program (QMB), Specified Low-Income Medicare Beneficiary (SLMB), and Qualified Individual program (QI), help Medicare beneficiaries of modest means pay all or some of Medicare's cost sharing amounts, i.e., premiums, deductibles and co-payments. This office will have a list of all qualified medicare beneficiary participants in the area. This new DHS office is located at 4885 Stage Road, Memphis 38128. This location is by appointment only; however, persons can walk into the lobby to pick up hard copies of application forms for DHS services and use self-service kiosks and phones to apply or address any problems with existing applications or programs; this site also has a drive through box to drop off applications or other needed documents.
