Description:

Provides prescription expense assistance for eligible households in Shelby County when funding is available. This program helps persons who are uninsured or under-insured and who meet other eligibility requirement.

Hours:

Mon - Fri 8:00 am - 4:30 pm

Intake Process:

Applicants must provide completed, signed application, copy of valid ID for head of household, copy of proof of household income for last 30 days, and copies of social security cards for all household members. Customers must also provide the actual prescriptions and/or refillable bottles. The program will provide up to $1000.00 per household/per program year.

CSA is accepting applications via online, mail-in, and drop box. To complete the online application or download the application, visit the website: www.shelbycountycsa.org. Mail-in or drop off applications with supporting documents are accepted at the following address: Shelby County Community Services Agency, 1188 Minna Place, Memphis, TN 38104.

Must be uninsured or have insurance that will not cover your prescriptions, meet income guidelines, be resident of Shelby County and provide documents to verify your status.
