Description:
Provides support to individuals at risk or affected by kidney disease. Programs include: transportation assistance to dialysis and transplant patients, free preventive kidney disease screenings, community education regarding kidney disease and transplant, emergency assistance to help kidney patients meet basic needs such as paying for utilities, rent, food and related supplies.

Intake Process:
Call or contact through website to request The Kidney Foundation free screening for kidney disease; for non-emergency medical transportation or financial assistance, individuals must be referred by the social worker at his/her dialysis clinic.

Kidney screenings are free to all; for other services, applicant must be a dialysis patient, transplant candidate or transplant recipient in Middle TN. Transportation assistance is only provided to applicants that are not eligible for TennCare.

Description
In person kidney screenings are available in Davidson and surrounding counties. Community organizations in the area can schedule screenings as part of their organizational or community events. TKF does not host screenings at the office or provide individual screenings. Self-guided education on kidney disease is available through the website
