Faith-based health clinic offering general medical services, behavioral healthcare and wellness care for people who are working but uninsured, or who have insurance, but cannot afford their copays or coinsurance. Provides limited prescription assistance to clinic patients.

Intake Process:
All patients seen by appointment for virtual or in-person appointments. Same day appointments are scheduled by a staff nurse on an emergent basis only.
Documents needed at each visit:
Current photo ID.
Proof of household income (IRS 1040 form).
Paystub (most recent and no older than 30 days) or notarized letter from employer.
If a student: school ID.
Be prepared to pay sliding scale fee (min $25 max $65) that is collected at each and every visit.
Be prepared to pay for ancillary services such as lab fees for pap smear/hpv testing, medication fees, and/or vaccine fees.
List of current medications.
New patients- all documents listed above, plus registration form.
New patients should arrive 30 minutes before scheduled appointment.
Program Fees:
Fees are determined by a sliding scale based on income. Average patient fee is $40. Unable to accept Medicare, Medicaid (TennCare), or Tricare.

Volunteer Opportunities:
Volunteers must be at least high school age and able to commit to a minimum of 10 service hours per month. Medical volunteers must undergo a skills assessment. All volunteers must sign a confidentiality agreement. Currently looking for volunteers to help with administrative tasks.
