Hours:
Mon, Tue & Thu 3:00 pm - 6:00 pm, Wed 1:00 pm - 4:00 pm Patients Seen;
Mon, Tue & Thu Noon - 7:00 pm, Wed 3:00 pm - 6:00 pm Office Hours
Intake Process:
Call (423) 581-4100 for Appointment - Must have an appoint-
ment - Will receive a Card to present when you go to the clinic.
