Description:
Provides help for pregnant teenage mothers from conception until the child reaches 5 years of age. First time initial visit is face to face, then one prenatal home visit, and one postpartum visit. Follow-up contact every month and can visit up to four times a month if needed.

Mon - Fri 8:00 am - 4:30 pm

No income restrictions. Service is based on need.
Eligibility:
Must be teenage pregnancy.
