Description:

Provides mobile unit and one permanent primary care site clinic (inside Catholic Charities of West TN) which dispenses acute and primary health care for homeless individuals and families. Diagnosis of medical problems and health needs, treatment and management of specific disease problems based on approved medical protocols includes dental care, mammograms, vision screening and free eyeglasses for eligible clients by Southern College of Optometry and treatment of minor injuries. All services provided at Baptist Operation Outreach are free of charge.

Beginning March 1, 2019, the agency opened a permanent clinic location (inside Catholic Charities of West TN site at 1325 Jefferson Avenue, Memphis 38104 on the main floor) open every Tuesday through Thursday from 8:00 am - 5:00 pm.

REGULAR Van schedule: Monday, Union Mission (383 Poplar Ave), serving homeless men; Tuesday, Wednesday & Thursday, 1325 Jefferson Avenue, CCWTN, serving homeless men, women, and children; Hours are Mon - Thu 8:00 am - 5:00 pm, except for the Second Wednesday afternoon of the month; on those Wednesdays, the van will be at the Shelby County Office of Re-Entry (1362 Mississippi Blvd) from 1:00 pm - 4:00 pm. Walk-ins accepted, but agency prefers that clients call for appointment ( 901-842-3167). NOTE: SITES MAY CHANGE, so PLEASE CALL FOR APPOINTMENT.

Eligibility:

Homeless men, women and children. Must be over 18 or accompanied by parent or guardian to receive treatment; teens on their own require referral from social service agency stating that they receive support from that agency and are legally responsible for themselves.
