Description:

Works to improve health and health care within 150-mile radius of Memphis by involving citizens, businesses, governments, schools, health care providers, media, fitness centers, and faith-based and other organizations in collaborative initiatives to address community health problems. Common Table Health Alliance is a multi-stakeholder, nonprofit, Regional Health Collaborative which serves the Mid-South. The Alliance's vision is to make Memphis one of America's healthiest cities by making improvements in the delivery of care. The organization aims to improve the quality of primary care, empower patients and caregivers, fight childhood obesity, reduce chronic diseases, and eliminate food deserts in low income neighborhoods. Current initiatives include: Let's C.H.A.N.G.E., Project Better Care, Memphis Breast Cancer Consortium, Health Literacy and Better Together Youth Health Alliance.
