Provides primary health care for children newborn and older at this site and referrals to other Memphis Health Center services at the main clinic location if needed. Services include dental, behavioral health, optometry, podiatry, ob-gyn and pediatrics. On site pharmacy available at main Crump Clinic only. This site moved from McLemore to Elvis Presley Blvd in February 2020

Program Fees:

Sliding scale fee, if necessary. Minimum charge is $25.00/visit. Pharmacy provides medicine on sliding scale fee. Also accepts most insurance carriers and TennCare plans including Americhoice and BlueCare (Volunteer State Health Plan), Medicare and Medicare Advantage, Unison Advantage (Americhoice), Health Springs, and Windsor. If no health care plan, referrals will be made.

Eligibility:

Must provide proof of family income and address for discounted care. Acceptable documents for proof of income include check stub, TennCare or Medicare card, Social Security Determination letter, and AFDC/Families First Eligibility letter or employment office eligibility letter. Acceptable documents for proof of address include utility or phone bill or mortgage statement.
