Description:
Provides HCHV, Healthcare for Homeless Veterans Outreach Program. Services include healthcare enrollment referrals, clinical assessment, referral to needed medical, mental health and substance abuse services, referral to job training/placement programs, referrals to VA and non-VA disability benefit programs, referral to short and long-term shelters and transitional home program assistance, referral to permanent housing, and referral to food, clothing and hygiene services. Another program, Justice Outreach, helps homeless veterans in the court system get into the probation program offered by the VA.

Mon - Fri 7:30 am - 4:30 pm

Call for help for homeless veterans or for information on Justice Outreach program.
