Description:
Provides medical and coordination services, care coordinator for children with special needs, statewide parental support network, medical assessments, TennCare/private insurance carrier and referrals coordination, diagnostic assessments/lab tests, dental/speech/hearing/physical therapy, and medical equipment. NOTE: Home visitation has been discontinued as of May 2020 due to the COVID-19 outbreak. Merged three departments to create this one in 2020: Children's Special Services, TennCare Community Outreach and HUGS (Help Us Grow Successfully).

Apply by phone or in person. CHANCE ( formerly called the Children's Special Services Clinic, HUGS & TennCare Outreach) is located at 814 Jefferson Ave, Room 154.
Program Fees:
No Fee
Eligibility:
TennCare eligible families, including pregnant women and high risk children under 5. TennCare covers eligible persons from birth to age 21 and children with special-needs from birth to age 21 who meet diagnostic/financial guidelines.
