Description:
Provides comprehensive case management for persons with mental illness, including assistance with housing, food, rent, utilities, transportation (bus or MATA Plus vouchers), medication management, some in-home psychiatric services and 24/7 crisis intervention. Through the PATH program (Project to Aid in the Transition from Homelessness), persons who are not already enrolled in services who are homeless with mental health issues are provided with a wide range of supportive services. Staff provides assistance with applying for SSI (Social Security Insurance) and other similar programs.

Mon - Fri 8:00 am - 5:00 pm

Call for pre-screening appointment
Program Fees:
Accepts Tenncare, Medicare or private pay. PATH program is free for eligible persons.
Eligibility:
Shelby County residents, adults and children, with mental health issues; to be eligible for the PATH program, persons must be uninsured, homeless or at imminent risk of becoming homeless with mental health issues and not currently enrolled in any human service programs.
