Reelfoot Rural Ministries - Dental and Eye Clinic

Description:

Provides dental services on the first and third Fridays monthly excluding holidays. Staffed by local volunteer dentists and dental assistants, services include extractions, fillings, and other minor procedures.

Hours:

Dental Clinic: By appointment only - call first

Intake Process:

Clinic requires proof of income (SS, SSI, AFDC, disability, pension, paycheck stub, food stamp papers), Social Security number for each household member, and driver's license number for household legal driver. Bring a list of all prescriptions strengths, dosages, and reasons for prescriptions to the clinic.

Program Fees:

$25.00 up-front fee does not guarantee treatment if medical conditions and prescriptions merit a medical consult prior to treatment and/or treatment needs exceed dental procedures offered. In these cases, fee is refunded.

Eligibility:

Low-income families or individuals without insurance in Dyer, Lake, and Obion counties in TN and Fulton & Hickman County, KY
