Description:

Provides an in-home personal assistant to help eligible persons to meet daily living needs such as personal maintenance, household services and meal management in their home. Home Respite and Attendant Care are also available. All services are available for persons with any type of disability.

Hours:

Mon - Thu 8:00 am - 5:00 pm; Fri 8:00 am - 3:00 pm

Intake Process:

Walk in or call for details. Referrals accepted from MCO, physician or the family

Program Fees:

Fees vary, but may be covered by the Medicaid Waiver or private pay

Languages:

Can provide Spanish speakers or staff with American Sign Language capabilities with prior notice

Eligibility:

Person of any age with any type of disability that needs assistance with daily living in the following counties in west TN: Carroll, Chester, Crockett, Decatur, Gibson, Hardeman, Hardin, Haywood, Henderson, Madison, McNairy and Weakley.
