Clinic Hours: Mon 8:00 am - 8:00 pm; Tue-Thu 8:00 am - 5:00 pm; Fri 8:00 am - Noon; Open one Saturday per month; inquire at clinic for dates/times
Intake Process:
Call for appointment, beginning at 7:30 am. Bring photo ID, insurance card, medicine and child's shot records to each visit. If self-pay, new patients need to bring proof of income. Pharmacies are closed for lunch each day from 1:00 pm - 2:00 pm. Refill requests from your pharmacy should be faxed to the clinic where you have been treated.
Program Fees:
Fees for users without insurance based on ability to pay. Medical care fees range from $25.00-$85.00 based on income and household size. Prenatal care package, $1200.00, includes all prenatal visits, ultrasounds on-site and physician delivery fee, but not lab work. Lab work fees are discounted based on income and household size. For services requiring a lab, the fees include the cost of the lab and the discounted cost of the procedure. Accepts TennCare and most private insurance plans.
