Description:
Provides support coordination for persons receiving the HCBS Medicaid Waiver services from the TN Department of Intellectual Disabilities (DIDD). Support coordination is provided by an Independent Support Coordinator (ISC) who develops, monitors and implements a Plan of Care to help persons choose paid services and access other supports to enhance their independence, productivity and integration into the community.
