Hours:

Monday-Thursday 8:00 am - 5:00 pm; Closed from 12:00 pm - 1:00 pm

Intake Process:

Application is on the website: www.friendsinneed.org.

Open Enrollment is Monday - Thursday from 9am-4:30pm at the clinic.

Required items for eligibility determination include: completed application, Social Security care, photo ID and Proof of Income for everyone in the household. Must have proof of who is living in the home.

Program Fees:

Sliding scale fee. 200% Poverty Guideline.

Eligibility:

Medical: Uninsured or underinsured.

Dental: Uninsured or underinsured.
