Provides free primary medical care and mental health care for working uninsured residents of Coffee, Franklin and Moore Counties. The agency now also accepts patients in these counties who are applying for disability, receiving disability without insurance, full time students, and those in transition between jobs (6-month allowance). Services include well woman and well man exams, check ups, sick visits and chronic disease management, specialty care referrals, telehealth visits and lab services. Mental Health services are on site only and include medication management by a psychiatric nurse practitioner and mental health counseling facilitated by a licensed counselor. Wednesday appointments are now held at a different site at 1914 McArthur Street, Manchester, TN 37355

Mon - Wed 8:00 am - 1:00 pm; Thu 8:00 am - 4:00 pm (closed 12:00 noon - 1:00 pm); Note: In addition to regular hours, the clinic is also open every 2nd Saturday from 9:00 am - 1:00 pm. Wed clinics are now held at the new Manchester site at 1914 McArthur Street, Manchester, TN 37355

Eligibility
Applicants should: *Live or work in Coffee, Franklin, or Moore Counties *Have a household member that works a minimum of 20 hours per week *Be uninsured and between the ages of 1-65 *Not have a total household income that exceeds 250% of current *OR: Be on Disability Benefits or applying for those *OR: Be a Full time student * OR: Be between jobs (up to six months limit) Federal Poverty Line
