Description:

Provides pediatric and adult dental care. Pharmacy located at Broad Avenue, Hickory Hill and Third St Clinics. Hickory Hill Clinic has full time Spanish speaking staff. Dental care includes exams, cleanings, sealings, extracts, limited periodontal treatment, restorations, dentures and partials, crowns and root canals.

Mon - Fri 8:00 am - 5:00 pm

Intake Process:

New patients should call for appointment, beginning at 7:30 am (appointments based on availability). Established patients can call for appointments 2 days in advance. Bring photo ID, insurance card, medicine and child's shot records to each visit. If self-pay, new patients need to bring proof of income. Pharmacies are closed for lunch each day from 1:00 pm - 2:00 pm. Refill requests from your pharmacy should be faxed to the clinic where you have been treated.

Program Fees:

Fees Vary. Dental care discounts range from 60%-75% depending on income and household size. For services requiring a lab, the fees include the cost of the lab and the discounted cost of the procedure. Accepts TennCare and most private insurance plans.
