Homeless individuals with a mental illness or co-occurring disorders; PATH Eligibility: Must be 18 years or older with a serious mental illness or co-occurring disorder, be homeless or at imminent risk of becoming homeless; and must live in: Benton, Carroll, Henry, Gibson, Lake, Obion, or Weakley Counties
