Fairview Housing - Appalachian Family Housing

Established in 2004, the Supportive Permanent Housing Program serves homeless families with children residing in the 8 Northeast Tennessee Counties / Eligible program participants meet weekly with an AFH case coordinator to receive guidance in obtaining affordable housing and then continue weekly meetings to establish, monitor and support each family's monthly and weekly goals in addressing employment, transportation, financial management, legal, education, healthcare and insurance needs / Participants must meet ALL of the following to qualify and apply: / 1- Must meet HUD's definition of homelessness: A family is defined as anyone who has legal custody of a child; and are considered homeless if they are living in places not meant for human habitation (cars, parks, abandoned buildings, etc) or staying in an emergency shelter, are fleeing domestic violence or have been served an eviction notice and will be living on the street without assistance / 2- At least one family member in the household is disabled: Applicants are required to provide documentation of a physical or mental impairment which includes, but is not limited to, diseases and conditions of orthopedic, visual, speech/hearing impairments, cerebral palsy, autism, epilepsy, muscular dystrophy, multiple sclerosis, cancer, heart disease, diabetes, HIV infection, developmental disabilities, drug addiction (must not be current illegal use) and alcoholism / 3- Must provide legal identification for all household members: Proper identification includes social security cards, birth certificates and documentation of custody of minor children in the family / 4- Must pass a background check: All family members 18 years and older are required to submit to a background check / 5- Must be willing to abide by the rules and expectations of the Supportive Housing Program / *Appalachian Family Housing's Supportive Housing Program is funded through the US Department of Housing and Urban Development (HUD)* /

Fairview Housing - Manna House - for Men

Manna House is a 40-bed facility providing safe and affordable transitional housing for homeless men focused on improving their current situation and therein their lives / All of the residents were chronically homeless, according to HUD's definition, and generally have addiction, legal problems and mental health issues / because the residents of The Manna House have accepted the powerlessness of their addictions and have made the conscious decision toward recovery, a high level of accountability and security exists at The Manna House / During their two-year program, residents attend twelve-step meetings and there is a zero-tolerance policy for alcohol and drugs /
